Indications:

Quiet XR is indicated for the treatment of acute and chronic psychoses, including schizophrenia, and bipolar disorder. This includes:

  • Treatment of manic episodes in bipolar disorder
  • Treatment of depressive episodes in bipolar disorder
  • Maintenance treatment of bipolar I disorder, in combination with a mood stabilizer, to prevent recurrence of manic, depressive, or mixed episodes.
Pharmacology

Quetiapine Fumarate is an atypical psychotropic agent belonging to the dibenzothiazepine derivatives. It acts as an antagonist at multiple neurotransmitter receptors, including serotonin 5HT1A and 5HT2, dopamine D1 and D2, histamine H1, and adrenergic α1 and α2 receptors. It has no appreciable affinity for cholinergic muscarinic and benzodiazepine receptors. The efficacy in schizophrenia is believed to be mediated through dopamine D2 and serotonin 5HT2 antagonism. Its antagonism of histamine H1 receptors may explain somnolence, and adrenergic α1 receptor antagonism may explain orthostatic hypotension.

Dosage & Administration of Quiet 50 XR Tablet

Dosage & Administration:

Schizophrenia:

  • Administer twice daily, with or without food.
  • Day 1: 50 mg, Day 2: 100 mg, Day 3: 200 mg, Day 4: 300 mg.
  • The usual effective dose range: is 300-450 mg/day, adjustable within 150-750 mg/day based on response and tolerability.

Manic episodes in bipolar disorder:

  • Administer twice daily, with or without food.
  • Day 1: 100 mg, Day 2: 200 mg, Day 3: 300 mg, Day 4: 400 mg.
  • Dosage adjustments up to 800 mg/day by Day 6 in increments no greater than 200 mg/day.
  • Effective dose range: 400-800 mg/day.

Depressive episodes in bipolar disorder:

  • Administer once daily at bedtime, with or without food.
  • Day 1: 50 mg, Day 2: 100 mg, Day 3: 200 mg, Day 4: 300 mg.
  • Can be titrated to 400 mg on Day 5 and up to 600 mg by Day 8.
  • Effective dose range: 300-600 mg/day.

Maintenance in bipolar I disorder:

  • Continue the same dose as in acute treatment with a mood stabilizer.
  • The dose can be adjusted based on response and tolerability.
  • Effective combination therapy dose: 400-800 mg/day.
  • Elderly: Use with caution, slower dose titration, and lower daily dose.
  • Children and Adolescents: Not indicated for use below 18 years.

Take Quiet 50 XR Tablet as advised by your physician. Swallow the medicine with a glass of water. Do not crush or chew the medicine. Quiet 50 XR Tablet should be taken at least one hour before meals or prior to bedtime.

Overdose Effects of Quiet 50 XR Tablet

  • Survival reported in overdoses up to 30 grams.
  • No specific antidote; consider intensive care measures.
  • Treat refractory hypotension with intravenous fluids and/or sympathomimetic agents (avoid adrenaline and dopamine).
Interaction
  • Use caution with drugs causing electrolyte imbalance or QT interval increase.
  • Co-administration with thioridazine, carbamazepine, or phenytoin increases the clearance of Quetiapine.
Contraindications

Quetiapine should not be used in the following situations:

  1. Allergy: If you are allergic to Quetiapine or any of its components.
  2. Drug Interactions: Avoid Quetiapine if you are taking:
    • Protease inhibitors for HIV (e.g., Atazanavir, Lopinavir).
    • Antifungal medications (e.g., Ketoconazole, Posaconazole, Itraconazole).
    • Certain antibiotics (e.g., Erythromycin, Clarithromycin).
    • Nefazodone is used for depression.
Pregnancy & Lactation
  • Safety during pregnancy is not established. Use only if benefits justify potential risks.
  • Unknown if excreted in human milk; avoid breastfeeding.

  • The risk of suicide and worsening depression persists until significant remission.

Store in a cool, dry place away from light. Keep out of reach of children. Do not use after the expiry date.
